A resident fell during the night and lay on the floor for more than 10 hours before being found the next morning. The overnight caregiver told investigators they "passed by" the room on rounds but never went in to look.

Oct 9, 2026
Three findings selected from AZDHS records currently available to the public. The inspections or enforcement actions may date from the past 36 months.

Medication records showed a resident receiving two insulin injections on days they received only one, and a pain medication documented as given six times while the facility was out of it and waiting on the pharmacy.
An employee who was sent to pick up a resident's prescriptions asked for the resident's bank PIN, and roughly $7,500 was taken from the resident's account. The resident reported it in October; management did not investigate or involve police until March.

Inspections or enforcement actions may date from the past 36 months. A finding reflects a point in time and may since have been corrected.
