Three findings selected from AZDHS records currently available to the public. The inspections or enforcement actions may date from the past 36 months.

Infant classroom rosters showed 10 children present when only 5 were actually there, and no roster existed for 8 one-year-olds on the playground.
Prescription and over-the-counter medications were given to five enrolled children without complete written authorization forms from parents or providers.
A resident left the facility without authorization, staff lost track of their whereabouts, and the facility failed to call 911 as required by its own policy.
The facility administered multiple medications to three residents without having physician orders on file to document why those medications were prescribed.
After a resident suffered a serious fall resulting in a cervical spine fracture requiring a collar at all times, the facility failed to update his care plan within the required 14 days.
