A resident in a home licensed for directed (memory) care walked out the front door without any caregiver knowing. The front-door alarm the home's own policy requires was not working, and no one had noticed.

Three findings selected from AZDHS records currently available to the public. The inspections or enforcement actions may date from the past 36 months.

While two caregivers were changing a resident overnight, one of them jabbed the resident in the stomach. A co-worker reported it to the nurse, but the accused caregiver was left on shift with residents for another six hours.
One caregiver was on duty for nine residents spread across two hallways. Call pendants chimed for more than 10 minutes at a time with no response because the caregiver was in another room and could not hear residents calling for help.

Findings are quoted or paraphrased from Arizona Department of Health Services inspection and enforcement records that are publicly available. Inspections or enforcement actions may date from the past 36 months. A finding reflects a point in time and may since have been corrected. WrenSong shares these to help families know what to ask, not to single out any provider.
