Two toddler classrooms were found with children present and no staff at all. The same facility had been cited for the same thing four times in the previous fifteen months.

Three findings selected from AZDHS records currently available to the public. The inspections or enforcement actions may date from the past 36 months.

A staff member struck a child hard enough to leave a visible adult-sized handprint on the child's back. Video showed the child leaving the restroom crying right after the staff member walked out.
A child was seriously injured around 8:30 in the morning. Staff called management and the parent, but no one called 911 until the parent arrived, saw the injury, and asked for an ambulance. The classroom was out of ratio at the time, and staff said they had raised the ratio problem before.
