What Does a Hospital Discharge Planner Do?
A hospital discharge can feel oddly fast.
One day your parent is receiving hospital care. The next, someone is asking where they will go, what equipment they need, and whether they can safely return home.
A discharge planner helps coordinate that transition. Depending on the hospital, the person performing this role may be a nurse, social worker, case manager, or another care-transition professional.
What can a discharge planner help with?
Their role may include helping the patient and family understand:
- Whether returning home is realistic
- What follow-up care is needed
- Whether home health or rehabilitation has been ordered
- What equipment may be required
- Medication and follow-up instructions
- Possible post-hospital care settings
- Transportation or transition logistics
Are they choosing an assisted living facility for you?
They may provide information or resources, but the family still makes the decision. This is an important distinction.
A professional helping with discharge planning has a different role from a commercial placement or referral company whose business model may involve receiving compensation when someone selects a particular provider. That does not automatically make one resource good and another bad. It does mean families should understand who the professional represents and how they are paid.
Questions you can ask
When someone gives you a list of care providers, ask:
- Is this a complete list or a preferred-provider list?
- Does the hospital have a financial relationship with any of these providers?
- Are you recommending these providers, or simply identifying available options?
- Where can I review licensing and inspection information?
- How quickly do I actually need to make this decision?
Pip's Tip
There is no awkwardness in asking “Can you help me understand your role in this decision?” Healthcare has a lot of titles. Families deserve to understand who is doing what.
