What Is the WrenSong Trusted Badge?

The WrenSong Trusted Badge identifies licensed care providers whose publicly available state licensing, inspection, and enforcement records meet WrenSong's published eligibility criteria.

Badge eligibility is evaluated consistently using the same criteria for every provider and reflects the state records available to WrenSong as of the date shown.

The badge is not a government certification, guarantee of quality or safety, or prediction of future performance.

Our Eligibility Criteria

Assisted Living Providers

  1. All enforcement actions fully closed and resolved. No outstanding corrective actions pending.
  2. No civil penalty of $1,500 or more within the preceding 36 months.
  3. No deficiency in the same category may be cited in two consecutive qualifying inspections that both occurred within the preceding 36 months.

    Two annual compliance inspections are considered consecutive even if another type of inspection occurred between them. Two inspections that immediately follow one another are also considered consecutive when at least one is a complaint inspection.

    Initial, monitoring, midyear, and modification inspections do not independently establish a repeat deficiency.

  4. No inspection finding within the preceding 12 months may state that it is a repeat citation or repeat deficiency of an earlier inspection.

    This applies whenever an inspection report's own finding language identifies a cited deficiency as a repeat of an earlier inspection, regardless of how many other inspections occurred in between.

A provider must pass all four to carry the badge.

Assisted Living Centers (11 or More Beds)

All four criteria above, plus:

  1. More than one inspection on record, if licensed before January 1, 2024. A Center licensed before this date is not eligible for the badge with only one inspection on file, unless that inspection followed a change of ownership.

A Center must pass all five to carry the badge.

Childcare Providers

All three criteria above, plus:

  1. No substantiated severe-incident finding in the preceding 24 months that includes any of the following: a medical-emergency fault (the wrong medication or dose given, medication given to the wrong child, an overdose, or a required medication not given), a significant child injury tied to a cited facility failure, staff physically harming a child, a facility failing to report suspected abuse or notify a parent, or a child eloping or going unaccounted for.

A provider must pass all four to carry the badge.

Every criterion above is evaluated from licensing, inspection, and enforcement records published by the Arizona Department of Health Services (AZDHS) , which is the state's own public regulatory record for licensed assisted living and childcare facilities. WrenSong does not use operator self-reported information, reviews, or any other source to determine badge eligibility.

Because eligibility depends on state records, a badge reflects what AZDHS had on file as of the evaluation date shown on a facility's report, not necessarily the moment you're reading this page.

Badge Integrity

Badge Integrity Requirement

A WrenSong badge is assigned only when a provider meets the badge eligibility criteria. Badge status may not be manually granted, removed, or modified independently of the documented criteria. Every badge decision is based on dated state records and can be independently verified.
